When a migrant domestic worker (MDW / FDW) arrives in Singapore to start under your Work Permit, the Ministry of Manpower (MOM) requires a pre-employment medical examination before her Work Permit can be issued. Miss the window, or fail the screen, and issuance stalls — or she may have to be sent home.

Passing the arrival medical does not replace later 6MEs. Treat them as two different compliance clocks.

What the pre-employment medical screens (high-level)
MOM states the examination is done by a Singapore-registered doctor and screens for four infectious diseases — tuberculosis, HIV, syphilis, and malaria — and checks whether she is fit to work. You may request additional tests if you have specific health concerns; discuss extras with the clinic / your EA (still no $ figures on this page).
Measles note (households with young children): if there are children in your household below 7 years old who are not fully vaccinated against measles, MOM requires you to ensure the helper has immunity against measles. Ask the clinic / EA how that check is documented for your household — do not invent a DIY certificate.
If you want a copy of the report, MOM says you can ask the doctor for one. Results that matter for WP issuance flow through the clinic’s submission channels to MOM — confirm status with your EA and FDW eService, not with SmartMaid.
Typical timing window — verify with MOM and your agency
Published MOM orientation (confirm live before you book):
- Send her for the medical within 2 weeks / 14 days from arrival in Singapore.
- She must bring the medical examination form for the tests.
- You can only get her Work Permit issued if she passes. Otherwise she will have to be sent home.

SIP vs medical (first-time helpers): Settling-In Programme (SIP) has its own shorter window (MOM: within 7 days, excluding Sunday and public holidays, for first-timers). Do not let SIP and medical compete for the same half-day without a plan — sequence both with your EA so neither deadline is at risk. Documents and items to bring
Exact lists vary by clinic and whether your EA pre-registers her. Build from the MOM medical examination form plus what the clinic asks when booking.
Usually prepare:

Remind her what the visit is for in plain language. Anxiety is normal on the first medical in a new country — a calm briefing helps more than a last-minute surprise.
If she is unfit or results are delayed (soft path)
Orientation only — follow MOM and your EA for your case:
- Ask the clinic what failed or what retest / follow-up is required and the expected turnaround.
- Tell your licensed EA immediately — WP issuance, IPA validity, and repatriation logistics sit with employer + EA processes, not with a marketplace.
- Do not assume a borderline result “will be fine for issuance.” MOM’s published position is clear: pass required to get the Work Permit issued; otherwise she has to be sent home.
- Document attendance date, clinic name, and who you spoke to.
- If you must end the hire, align cancellation / repatriation steps with MOM and your contract — soft adjacent context only: When a helper takes overseas leave and does not return (unique #1) is about a different failure mode; use it only for related non-return facts, not as medical advice.

Transfer helpers vs overseas / new arrivals
The classic pre-employment medical clock (within 2 weeks of arrival in Singapore) is written for helpers who enter Singapore to start work under the IPA / WP path.

Transfer helpers already in Singapore: she may already hold a Work Permit and an ongoing 6ME history. Do not assume the identical “14 days from wheels-down” story applies unchanged. Confirm with your licensed EA and MOM whether a fresh pre-employment medical is required for this transfer, or whether current medical / 6ME status and transfer WP steps are enough. MOM’s published guidance is within 2 weeks / 14 days from arrival. Verify the live page before you plan — do not rely on a blog alone.
Who pays?
The employer bears medical examination costs related to employment requirements. Confirm the clinic’s fee when you book; this page quotes no $ amounts.
Can she start work before the medical?
Work Permit issuance requires a pass. Follow MOM / your EA on what she may do on IPA / temporary arrangements before issuance — do not invent a “start chores on day 1 anyway” rule here.
Does a transfer helper need this medical?
